Use the application forms below to sign up for Klick^sm Banking, or make changes to an existing Klick^sm Banking account.

Fill out these forms on your computer, then print them using your printer. Sign the printed form and return it by mail or fax to the address or fax number listed on the form, or bring it to your nearest Community Bank & Trust Co. branch office.

New Klick^sm Banking users may fill out the Klick^sm Banking Application/Agreement ( pdf, 274kb).
Existing Klick^sm Banking customers may add, change or remove accounts by using the Klick^sm Banking Maintenance Application ( pdf, 262kb).
